At Omega Physical Rehab, we prioritize your privacy and ensure the confidentiality of your medical records. Please follow the steps below to request medical records based on your specific needs. If you are a patient, you can access your records online:

If you are a patient, you can access your records online using our WebPT patient portal. Through patient portal, you can view, download, and print your records. Simply click here to access WebPT patienand follow the instructions to log in. 

Request Records to be Sent Elsewhere

If you would like us to send your records to another party, please complete the Release of Information Authorization Form. First, download and fill out the form completely, ensuring all required fields are accurately completed. Include a copy of your valid photo ID (e.g., driver’s license, state ID, or passport) with your request form. Submit your request by emailing the completed form and a copy of your ID to medicalrecords@omegaphysicalrehab.com or faxing it to 908-548-8678 or you can mail the completed form and ID to:

Hospitals/Physician Offices or Third Party Requests:

If you are a hospital or physician’s office or a third party requesting medical records of someone other than yourself, please follow these instructions. Download and complete the Medical Records Request form which can be found below and attach a signed Patient Consent form. Submit the forms by emailing them to medicalrecords@omegaphysicalrehab.com or faxing them to 908-548-8678.

Fast Track Your Records Request

We are committed to protecting the security and privacy of patient information. All medical records are confidential and protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Records will only be released with written authorization. We are not able to take verbal requests over the phone for medical records.

Processing Time

Please allow up to 14 business days for your request to be processed. We’ll let you know as soon as your records are ready!

Hours of Operation

Our Medical Records Department is open from 9 a.m. to 4 p.m. EST, Monday to Friday, excluding major holidays.
